By clicking the checkbox, I hereby agree to the following:

1. To pay to the SAHL the total sum indicated on the web site at the time of registration;

2. To abide by the SAHL playing rules including the Code of Discipline;
3. To produce my birth certificate or other official proof of age in the event that it is requested by the SAHL or a league official;
4. To outfit myself with the proper protective equipment as specified by Hockey Canada and SAHL league rules;
5. To reimburse the SAHL immediately, in cash, the full face value of my cheque plus any applicable bank charges and an administration fee of $20.00 should any cheque which I tender be rejected by the bank;
6. To report immediately my decision to withdraw or resign from the League for any reason (refund of fees will be calculated from the date such resignation is submitted to and accepted by a member of the SAHL Management Team).
7. To acknowledge that the SAHL has purchased CARHA Hockey insurance on my behalf and that I accept all risks arising from my participation in the SAHL and release the SAHL (as represented by the SAHL Management Team) from any injury or damage claim or action
